Separate sealed BIDS for the construction of the Train Depot Walking Track Rehabilitation and Expansion Project (2020 SMLPC Project No.SM-19-885) will be received by the Town of Lake, herein called the "OWNER", at the office of Lake Town Hall, located at 100 Front Street, Lake, Mississippi 39092, until 7:00 p.m., Local Time, on Tuesday, November 2nd, 2021, at which time they will be publicly opened and read aloud. Bids received after this time will not be accepted and will be returned unopened.
Contract time is 120 consecutive calendar days. Liquidated damages in the amount of $200.00 will be assessed for each consecutive calendar day thereafter.
Bids must be submitted on the standard form of bid schedule and must include a Bid Bond in the amount of 5% of the Bid. The successful bidder will be required to execute the standard form of Contract and Agreement, together with a 100% Performance Bond and a 100% Payment Bond, within 10 days after formal award of the contract.

Copies of the CONTRACT DOCUMENTS may be obtained at the office of Clearpoint Consulting Engineers, P.A., located at 6652 US Highway 98, Hattiesburg, Mississippi 39402 upon payment of $100.00 for each set, NOT to be refunded.
Bids will be received only from experienced contractors who have completed similar projects as described in these contract documents. A letter shall accompany his bid listing five successfully completed projects of this type. Bidders must be qualified under Mississippi Law and show a current Certificate of Responsibility issued by the Mississippi Board of Public Contractors establishing classification as to the value and type of construction work on which he is authorized to bid.
Minority and women's business enterprises are solicited to bid on this contract as prime contractors and are encouraged to make inquiries regarding potential subcontracting opportunities, equipment, material and/or supply needs.

All projects funded by the Local Governments and Rural Water Systems Improvements Board (Board) under the Drinking Water Systems Improvements Revolving Loan Fund (DWSIRLF) undergo a review to determine whether the proposed project will have a significant impact on the environment. In this determination, it is assumed that all the facilities and actions recommended in the facilities plan will be implemented. The facilities plan for this project is "2020 Water Systems Improvements" by Engineering Service Consulting Engineers.
This project consists of replacing approximately 156,000 linear-feet of existing 1-inch, 2.5-inch, 3-inch, 4-inch, 6-inch, and 8-inch polyvinyl chloride (PVC) water pipe with 4-inch, 6-inch, and 8-inch SDR-21 (200psi) PVC water mains.
The Board has determined that the proposed project meets the criteria for receiving a categorical exclusion from further environmental review. However, this determination can be revoked if significant adverse information becomes available. REQUEST FOR
QUALIFICATIONS TO
PROVIDE ARCHITECTURAL SERVICES
This is a Request for Qualifications to provide Architectural Services for a proposed U.S. Department of Commerce Economic Development, Administration (EDA) project for building improvements to the Scott County-Forest Coliseum for the City of Forest and the Scott County Board of Supervisors. The Architect will be responsible for preparing cost estimates for the application preparation.
Interested parties are invited to submit qualifications (8 copies) in accordance with this request to the City of Forest/Scott County Board of Supervisors, c/o City of Forest, P. O. Box 298/120 S. Davis Street, Forest, MS 39074, not later than 4:00 p.m. on October 18, 2021. Submissions shall be marked on the outside as "Qualifications for Architectural Services/ EDA". Information concerning this request may be obtained by calling Jenifer Buford (601) 683-2007.
The contract will be awarded to the responsible offeror whose qualifications are within the competitive range and determined to be the most advantageous to the program, with knowledge, skill, experience, and other factors considered. The factors to be considered in evaluation of qualifications and their relative importance are set forth below.
The Contractor shall perform all the necessary Architectural services to properly carry-out the activities in the project, in accordance with State and EDA prescribed rules, regulations, policies, and State law. The project includes the following activities.
1) Preparing project cost estimates for the application preparation to EDA.
2) Prepare plans and specifications for project including all services required for design and Architectural phases of project.
3) Construct and distribute bid packets (ensuring that all Federal and State requirements are met in contract preparation).
4) Assist in bid opening and prepare bid tabulation
5) Conduct pre-construction conference with contractor, and staff representatives, documenting files with minutes of meeting.
6) Conduct work in-progress inspections giving periodic reports to the City and approving any and all partial payment requests.
7) Provide all services necessary for execution of the project including consultations, surveys, soil investigations, supervision, travel, "as built" or record drawings, and incidental costs.
8) Provide the recipient, EDA, Comptroller General of U.S. Dept. of Inspector General, or any authorized representatives access to all records of the project.
9) Maintain all records for 3 years after project is closed.
The contract will be on a fixed price basis. Those desiring consideration should submit qualifications by the time and date above stated and must include the following:
1. Qualifications- List of qualifications of each staff person assigned to project. (40 points)
2. Experience- Information regarding the experience of the firm. This information should include types of project activities undertaken. (40 points)
3. Capacity for Performance-Identify the number and title of staff available to be assigned to provide services. (20 points)
All submissions will be rated on the above system to determine the best offeror.
Qualifications will be reviewed by the Selection Committee using the above selection criteria. The Committee will assign points to each criterion based on the content of the submission. Negotiations will be conducted to determine a mutually satisfactory contract with the firm receiving the highest accumulated points, as rated by the Committee. If a mutually satisfactory contract cannot be negotiated with the firm, the firm will be requested to submit a best and final offer in writing; and if a contract cannot be reached after the best and final offer, negotiations will be initiated with the subsequently listed firm in order of rating. This procedure will be continued until a mutually satisfactory contract has been negotiated. In addition to reaching a fair and reasonable price for the required work, the objective of negotiations will be to reach an agreement on the provisions of the proposed contract including scope and extent of work, and other essential requirements.

RESOLUTION ESTABLISHING THE TAX LEVY FOR THE CITY OF FOREST, MISSISSIPPI, AND FOR THE FOREST MUNICIPAL SCHOOL DISTRICT OF SCOTT COUNTY, MISSISSIPPI.
WHEREAS, according to law, the personal property assessment roll and the real property assessment roll of the City of Forest, Mississippi, for the year 2021 have been equalized and the personal property assessment roll and the real property assessment roll of the Forest Municipal School District of Scott County, Mississippi, for the year 2021 have been equalized; and
WHEREAS, the Mayor and Board of Aldermen are authorized by law to fix the tax levy for said City of Forest and said Forest Municipal School District on this the 7th day of September 2021, and it appearing that said tax levy should be made at this time.
NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND BOARD OF ALDERMAN OF THE CITY OF FOREST, MISSISSIPPI, as follows:
SECTION I: The following municipal ad valorem tax levy for the City of Forest, Mississippi, and the ad valorem tax levy for the Forest Municipal School District of Scott County, Mississippi, for the fiscal year next succeeding, be, and it is hereby made and fixed on each dollar of valuation under the authority of Section 21-33-45, Mississippi Code of 1972, as follows:
